We continue to work on improving the quality of our patients’ medication lists at discharge. We’ve already started making improvements to specific medication orders, and have started sending out quality data to all division leaders. Here’s our latest quality data snapshot:
Our next steps include making a change to how clinicians view the orders tab in CIS. Currently, on the orders tab, no home medications display, making them essentially “out of sight, out of mind” during an inpatient stay. We will be adding home medications to the orders tab to make the more face up. They will be differentiated from inpatient orders by the icon. Inpatient have a little “hospital” icon, whereas prescriptions have a pill bottle and historical medications have a gray scroll.
Other potential future improvements include changes to the med rec defaults, addressing MA/RN/MIC scope of practice, and working on ambulatory and ED processes.
